I understand that if my assignment should lead to a full-time permanent position, my employer will be obligated to pay an additional fee to Accurate Personnel/ Temporaries LLC. to be determined at the time of hiring. If my prospective employer refuses to pay the necessary fee, I agree not to accept a permanent position with that employer. If I do accept employment or if I accept employment with another temporary service at an company referred to me by Accurate Personnel/Temporaries LLC. I agree to personally guarantee to Accurate Personnel/Temporaries LLC payment if the amount due for its fee and expenses including attorney’s fees arising out of my employer’s refusal to pay the necessary fee. 
    The information supplied was submitted by me and all information is true and correct to the best of my knowledge. I understand that any material misrepresented will be cause for immediate termination. I also understand that the information supplied by me regarding my employment history, education (authorization includes release transcripts), credit, criminal history, motor vehicle record, residence history, workman’s compensation claims, and personal references will be utilized as part of the processing procedures and that a background investigation will be conducted to verify the validity of the information submitted and to develop information concerning my character, general reputation, personal characteristics, and mode of living. 
    I herby authorize Accurate Personnel/Temporaries LLC to make a thorough investigation of my past employment, education, and activities and I release from liability all persons, companies, and corporations supplying that information. I release and indemnify Accurate Personnel/Temporaries LLC and/or On Line Screening Services, Inc. against any liability that might result from making such investigation.
